This agency is made up of three Divisions: Rehabilitative
Services, Visual Services, and Disability Determination. The
agency's mission is to provide opportunities for individuals with
disabilities to achieve productivity, independence and an enriched
quality of life. In order to locate Vocational Rehabilitation
providers and vendors nearest you contact your Vocational
Rehabilitation Counselor.

An individual is eligible for vocational rehabilitation services
if he/she:
 | Has a physical or mental impairment which results in a
substantial impediment to employment; |
 | Can benefit in terms of employment outcomes from vocational
rehabilitation services; |
 | Requires vocational rehabilitation services to prepare for,
enter, or retain gainful employment. |
Individuals with the most severe disabilities
receive priority in obtaining services.
Employment Services for persons with disabilities include:
 | Counseling and guidance to help each person attain
occupational goals |
 | Medical care and durable medical equipment needed to remove
or reduce limitations resulting from disability |
 | Prosthetic appliances such as limbs, braces, hearing aids,
eyeglasses and other devices when needed to increase work
capacity |
 | Prescribed adaptive technology to increase consumer's
abilities to function successfully |
 | Tuition and fees at trade or vocational schools, colleges or
universities and/or on-the-job training |
 | Books and other related training expenses |
 | Pre-vocational training through community based
rehabilitation facilities |
 | Financial maintenance, transportation, tools, equipment,
licenses, or initial inventory for starting businesses |
 | Job placement and on-the-job follow-up |
Consumers may be required to participate in the cost of these
services, depending on their income levels.

Developmental Disabilities Services Division, (DDSD) provides
specialized services for individuals that are diagnosed with a
developmental disability. In order for an individual to qualify
for services their full scale IQ must be below 75. Services
provided are In Home Support Waiver and State Funded Services,
Group Home and Employment Services, as well as financial
assistance through Family Subsidy Assistance Payment program. To

Workforce Oklahoma is an agency that works in conjunction with Oklahoma Employment
Security Commission. The agency assists applicants as well as
business to identify potential candidates and employment

OKLAHOMA LEAGUE FOR THE BLIND

The Oklahoma League for The Blind is a manufacturing facility
that manufactures garden and fire hoses, and wooden wheel chocks
as well as packaging for food rations and first aid kits for the
United States Military.
The League must have a 75% to 25% ratio of employees with 75%
being blind or visually impaired (legally blind) and 25%
non-disabled or other disabilities. Employees must be 18 years
or older.

Dale Rogers provides vocational services for individuals that
experience a developmental disability. In order for an
individual to receive services they must be diagnosed with Mental
Retardation. Services provided are Sheltered
Workshop and Community Integrated Employment Services as well as
they have a special needs program for individuals that require
additional support and supervision. Dale Rogers also offers a
school to work transition program for high school juniors and
seniors. Individuals can be employed in sheltered workshop
activities at Prism Place. They also provide employment in areas
of mobile workforce/enclaves, lawn and landscaping maintenance
crews, supported and job coach services as well as contract
employment opportunities at Tinker Air Force Base for custodial
and food preparation services.
Dale Rogers Training Center has several programs including;
 | School-to-work Transition Programs prepares juniors and
seniors to compete in the job market. |
 | Sheltered Workshop provides in house training and paid work. |
 | Special Needs Program offers a supportive, slower paced
environment. |

 | Supported Employment assists individuals in finding and
retaining jobs in the community. |

food service and custodial contracts providing well above
minimum wage and benefits.

Reliant provides residential living services, (ICFMR), Intermediate Care
Facility services and vocational assistance as well as and
sheltered workshop services to developmentally disabled
individuals. In order for an individual to receive assistance
they must be 18 years of age and be diagnosed with mental
retardation as well as they must achieve an intelligence (IQ)
score of 72 and below.
JOB ACCOMMODATION NETWORK
(JAN) is a international consulting service that provides
information about job accommodations and the employment of
people with disabilities. For additional information contact

A service of the President's Committee on Employment of People
with Disabilities the Job Accommodation Network (JAN) is an
international consulting service that provides information about

WORKFORCE INVESTMENT ACT/ TICKET TO WORK
Former President Clinton signed the Ticket to Work and Work
Incentives Improvement Act on December 17, 1999. The Act was
designed to bring about positive change to the lives of individuals
with disabilities by providing them more choices in the training
and support they need to return to work.
Many SSI and SSDI Beneficiaries in Oklahoma have received a
Ticket to Work since the program began in February 2002. Those
SSI/SSDI recipients that have not received a Ticket to Work may
apply for one by calling Maximus at 866-968-7842. For more

Supported employment and retention programs
combine paid work opportunities with individual support (such as
job coaching) to help individuals with severe disabilities gain and
maintain competitive employment. These programs are designed for
individuals that have often been unserved or under-served by
traditional vocational programs. For further information, contact

Is designed to serve developmentally disabled individuals.
Individuals must possess a full scale IQ of 75 and below in
order to receive services. For more information contact (DDSD),

Individuals must have a diagnosis of Mental Retardation in order
to receive state funded services. In order for an individual to
be certified, psychological testing from a licensed
psychologist, (Ph.D.) or educational testing from the recipients
school must accompany application for (DDSD) services. Testing
has to be conducted with in the last five years in order to
satisfy eligibility criteria. Test results must reflect a full
scale IQ of 75 and below for eligibility determination. A
certification letter will be provided to the applicant for
verification.
GALT FOUNDATION
A temporary staffing service that provides employment
assistance to disabled individuals. The Galt Foundation is also
a vendor supporting the state use program. For additional

RES-CARE OKLAHOMA
Res-Care is an agency that provides a variety of services to
developmentally disabled individuals. An individual must be
diagnosed with Mental Retardation and possess a full scale IQ of
75 and below in order to participate in activities. The services
an individual can receive is IHSW, In Home Support Waiver
Services through (DDSD) to include (HTS) Home Habilitation
Training Services and (HM) Homemaker Services, Respite Care, and
Transportation Services, as well as Vocational and Group Home

INDEPENDENT OPPORTUNITIES (IOI)
(IOI) Provides specialized Services for developmentally disabled
individuals. An individual must be diagnosed with Mental
Retardation and be certified through Developmental Disability
Services Division, (DDSD) in order to receive assistance. Services include HTS, Home Habilitation
Training and Home Maker Services, as well as Transportation and

ALLIED COMMUNITY SERVICES
Allied provides services for individuals that experience a
developmental disability as well as have a primary diagnosis of
Mental Retardation. Individuals can receive in home
support waiver services as well as employment assistance. Allied

EDMOND ASSOCIATION FOR RETARDED CITIZENS,
(EARC)
(EARC) provides services to developmentally disabled individuals.
